Gardner Homeless Project

Gardner Family Health Network - Health Care for the Homeless Project

Healthcare for the Homeless provides primary medical care, social services, health education, contracted services, referrals. Services are provided on an outreach basis to shelters, churches and drop in centers. Services are also provided at community events and health fairs.

Valley Homeless Healthcare Program

The mission of the Valley Homeless Healthcare Program is to be the “safety-net for the safetynet” servicing vulnerable populations who cannot or will not seek care in a conventional healthcare setting. By providing care that integrates primary medical, substance abuse, and mental health services, the program hopes to increase the number of homeless persons utilizing preventive care services, decrease inappropriate emergency department visits, and offer hope and options for an impoverished and disenfranchised population."
